Replacement Cards

If your active Arts Access card is lost or misplaced, you can request a replacement by submitting the form below.
If your card is expired, please apply for a new pass here.

If your address has changed, please let us know in the form. Replacement cards typically arrive in 2–4 weeks. Your new card will have the same expiration date and member ID.
